The only way to know for sure is to have a professional look at it. Redness and swelling are part of the piercing process; you are putting a foreign piece of metal in your tongue so your body is going to try to combat it. Read your after care sheet and remember to rinse after you eat or drink anything. If you are a smoker make sure that you rinse after that as well. An infection site will be red and tender so some people might get confused with the normal healing process that goes along with a tongue piercing.
